So have I. Not fun on a distant co-lo box in the middle of the night. 
Nowadays I always change the root password to something stupid, start up 
telnetd, upgrade sshd, and if all is well, change the root password back, 
and stop telnetd. Even though I haven't had the problem with the init 
scripts killing sshd children since, it lowers the stress level 
considerably.
